Apply for Death Certificate

Procedure involved to get this service: Application along with necessary documents and prescribed fee should be submitted to VA and collect the certificate from VA.

Documents to be enclosed with the request:

1. ID PROOF
2. ADDRESS PROOF
3. DEATH DETAILS (MOTHER NAME, FATHER NAME,PLACE OF DEATH DATE OF DEATH)
4. FIR (UN NATURAL DEATH)
5. POSTMORTEM REPORT (UN NATURAL DEATH)
6. DEATH CERTIFICATE FROM MEDICAL OFFICER, INCASE OF DEATH @ INSTITUTIONAL DEATH
7. HEADMAN OF THE HOME, IN CASE OF DEATH.
