Birth Certificate

Procedure involved to get the certificate is Application along with necessary documents and prescribed fee should to VA and collect certificate from VA.

Documents to be enclosed with the request

1. ID Proof
2. ADDRESS PROOF
3. BIRTH DETAILS (MOTHER NAME, FATHER NAME,PLACE OF BIRTH DATE OF BIRTH)
4. BIRTH CERTIFICATE FROM MEDICAL OFFICER, FOR INSTITUTIONAL BIRTH
5. REPORT FROM ANM, IN CASE BIRTH @ SUB CENTERS/ VILLAGE HOSPITALS
6. HEAD OF THE HOME, IN CASE OF HOME DELIVERY
