Disability Certificate

Procedure involved to get this service: After getting certified by the concerned Medical Authority in the prescribed format the identity card will be countersigned registered and issued by the concerned District Welfare Officer with in 7 days.

1. DISABILITY CERTIFIED BY A MEDICAL AUTHORITIES IN A PRESCRIBED FORMAT IN THE IDENTITY CARD
