Procedure involved to get this service Application along with necessary documents and prescribed fee should be submitted at the nearest AJSK Centres.

Documents to be enclosed with the request

1. APPLICANT SHOULD MENTION THE TOTAL INCOME OF THE FAMILY AND THEIR SOURCES IN THE PRESCRIBED APPLICATION FORM.
2. GOVERNMENT/PRIVATE EMPLOYEES SHOULD ENCLOSE THE SALARY CERTIFICATE IN ORIGINAL OR COPY ATTESTED BY GAZETTED OFFICER.
3. RATION CARD OR ELECTION IDENTITY CARD.
4. ATTESTED COPY OF PAN CARD OR DRIVING LICENSE.
